After review of the report, the Gallery elected to have us perform a hydrostatic test of the piping sections instead of directing the Contractor to repair the work. Only the gross weld defects were replaced after the hydrostatic test.

Example: Saint Elizabeth Hospital, Washington, DC. We were tasked with running two 50,000 lb. steam boilers in temporary make shift shelters. The boilers operated 24/7 at full capacity during the winter months with no redundancy, and required 100% city water make-up with no returns. The third boiler failed under another contractor and the hospital elected not to replace it. The boilers supplied the entire campus. There was no spare or reserve capacity. Through careful planning and execution we supplied the hospital and outlying campus with an uninterrupted steam supply for seven years. The boiler internals were clean with no scale at the end of the Contract. This was a difficult task. Water treatment was a high priority with 100% city water make-up and no returns. Proper spare parts and emergency procedures also became a high priority with no spare boiler.

After investigation, boilers were re-rated to meet actual heat release rates and lower boiler casing outer skin temperatures to within Industry Standards. Manufacturer altered new boilers to meet heat release rates as indicated in specifications. After a thorough investigation we found a lack of full penetration welds in the main 14 and 8 inch steam piping in the plant. Removed selective weld sections for bend testing. Passed. Performed hydrostatic test on steam piping. Passed. Code questions resolved. Issue resolved.
Example: Saint Francis Hospital, Loss Incident Investigation of a unitized, dry-pack, 1500 KVA distribution transformer. After a thorough investigation, failure was attributed to vibration of the windings leading to abrasion and insulation breakdown resulting in an arcing ground fault in one primary coil which escalated to a full phase to phase fault condition.
Example: East Carolina University Boiler, Chief Engineer requested assistance in determining cause(s) of excessive vibration of Forced Draft Fan. Investigation revealed damper closing before variable speed drive slows down leading to air reversals within the duct creating pulsations leading to vibration and failed bearings in the forced draft fan. Initiated time/rate function change to close damper at 20 Hz or below on shutdown and pre-open damper to a minimum position on start-up. Issue resolved.
Example: Boiler Condition and Useful Life Study for Simkins Industries, Inc. Immediate repairs or replacement of boiler was recommended. Three options were submitted with estimated costs for each.
Example: Loss Prevention Reports at the following locations. These reports included risk assessment and inspection of all mechanical and electrical systems in the facility. Critical/important exposures and safety issues were identified and remedial action was taken through written recommendations to minimize or prevent a future loss event from occurring. Equipment includes boilers, chillers, transformers, switchgear, fire protection, process machinery and auxiliary equipment.
